Reporting Requirements:
HIVDS: Immediate phone notification followed by submission of Adult HIVDS Confidential Case Report Form DSHS 13-13674 within seven calar days to your local reporting authority.
Syphilis, Chlamydia, Gonorrhea, Chancroid: Submit a completed Confidential Report of Sexually Transmitted Diseases Form STD-27 along with the reportable cases within seven calar days.
Exemptions: Reporting under Texas law and Administrative Code is not subject to HIPAA restrictions.
Adult Cases:
Acute HIV: Call your local reporting authority within one working day.
Non-Acute HIV DS: Submit the Adult HIVDS Confidential Case Report Form DSHS 13-13674 to your local authority within seven calar days.
